Breast Augmentation is a surgical procedure to enhance the size and shape of a woman’s breast. This is now the most common surgical procedure performed in the United States each year.
The Breast Augmentation implant procedure is designed to increase the breast size and enhance the overall bodies appearance to a beautiful and sensual look. Your consideration of breast implants, for reconstruction or for augmentation, should be based on realistic expectations of the outcome. Individual results vary based on individual factors, such as overall health, chest structure, healing capabilities, and prior breast surgeries.
The possible incision locations include:
- the crease below the breast (inframammary crease)
- around the areola (peri-areolar)
- under the armpit (transaxillary)
- at the superior belly button (transumbilical)
Implant Placement Location:
- below the muscle, submuscular (subpectoral)
- under the gland (subglandular)
The implants can be placed either under the chest muscle or directly under the breast tissue. Placement considerations include the anatomy of your breasts, volume of breast tissue, size of desired implants, exercise activities (upper body strength and conditioning sports such as weightlifting, tennis, and volleyball. There is a minimal interference with mammograms, especially with the displacement technique of the implant during the mammogram study.
Implant Types:
- Saline
- Silicone round implants
- Firm Silicone (“gummy bear”)- shaped implants
There are several implants available at the present time. The implant used with less frequency over the past several years in breast plastic surgery operations in the United States is made of a silicone rubber implant filled with a saline solution.The benefits of a saline implant is that if it ruptures the fluid is absorbed by your body within a few days and that it can also be placed through the umblicus or belly button.
Saline Implant Limitations:
- less natural feel (firmer)
- less natural appearance
- higher chance of rippling, especially at edges
Silicone Breast Implant Advantages:
- more natural feel (soft and supple)
- more natural appearance
- less risk of ripplilng
- may have reduced risk of capsular contraction
Silicone breast implants are the preferred implant due to the above advantages. Ray Khan uses a Keller Funnel during silicone implant placement to reduce the risk of bacterial contamination by 35 times, reducing the contraction risk as well as reducing the length of the breast incision (usually 40 mm only or 11/2 inches) and shortens the surgical time (less than 15-20 minutes typically for each breast.
Ray Khan also uses a wetting solution of an anesthetic (lidocaine) with an agent to inhibit bleeding, adrenaline (epinephrine), which decreases postoperative discomfort, especially in the first 24 hours when the discomfort is the greatest as well as reducing the risk of capsular formation by minimizing bleeding during the procedure.
Breast Implant Incision Study:
Ray Khan is also completing study evaluating the effect of CO2 laser skin resurfacing to the breast implant incision and has found that laser skin resurfacing within the first 4-6 weeks after surgery improves the incision appearance. The addition of “scar gels” also has been shown to improve the appearance of the breast implant incision.
Under certain conditions, such as reconstructive surgery, and various anatomical considerations autologous fat transfer (ATF) or Natural Breast Augmentation can be used instead. Many patients prefer fat grafting to the breast with the addition of Platelet rich plasma (PRP) and or stem cells to implant surgery, especially in those who have undergone breast cancer surgery. Studies have shown a reduced complication rate and better fine tuning enhancements using fat compared to implant surgery.
Breast Augmentation Indications include:
- To enhance the body contour of a woman who feels her breasts are too small
- To correct a reduction in breast volume after pregnancy
- To balance a difference in breast size
- To reconstruct a breast after breast surgery
